We instinctively know that a more connected and collaborative primary health care system will improve health equity and health outcomes for all Western Australians. This plan sets out our priorities for the next three years and a path towards our vision of a more connected primary health care system – a system that minimises barriers to accessing primary health care and ensures those with the greatest health needs are cared for as close to home as possible.
The Strategic Plan was developed through consultation and iteration to ensure it is fit for purpose. It was developed by the Executive then tested with managers, our Commissioning for Better Health Partnership Group and the Chairs of our Consumer, Clinical and Aboriginal Health and Wellbeing Advisory Committees before being endorsed by our Board in July 2020.
