Access to multidisciplinary care across Perth and the Midwest is supporting smaller and solo general practices to better manage chronic conditions and improve patient outcomes.
Following a comprehensive and collaborative process, WAPHA has successfully commissioned ‘Multidisciplinary Teams (MDT) in General Practice’ with PDC Health Hub in the metropolitan area and Central West Health & Rehabilitation in the Midwest to deliver these services. This marks an important step forward in strengthening accessible, coordinated primary care.
The activity will bring together health professionals such as nurses, podiatrists, dieticians and physiotherapists to work alongside selected general practice teams. Patients at participating practices will be able to access services including face-to-face and telehealth care, case conferencing and coordinated support tailored to their needs.
Importantly, this model is designed to operate flexibly across practices, ensuring services are delivered where they are most needed and sustained over time.
WAPHA’s Executive General Manager – Primary Care, Tony Mottershead, said the introduction of multidisciplinary teams in general practice represents an important step in strengthening delivery of care in the community.
“By partnering with providers and general practices, we are improving access to coordinated person-centred care, particularly for people living with chronic conditions. This model helps reduce pressure on GPs while enabling clinicians to work to their full scope of practice, ultimately supporting better outcomes for patients.”
Through this program, WAPHA will be working with participating practices and providers to embed a sustainable, team-based model of care. By improving access to multidisciplinary support, the initiative aims to reduce barriers to care, enhance coordination, and support better health outcomes across WA communities.
