Palliative Care Champion project in general practice – Request for Proposal (RFP) opening 5 December

For the third year running, WAPHA will be providing a small grant to practices across WA to enable a member of their team to undertake the role of Palliative Care Champion.

The intention of this project is to strengthen current approaches to advance care planning and screening for unmet palliative care needs through routine health assessments and chronic disease management care. WAPHA has successfully conducted this project with positive outcomes for the 13 practices who have already participated.

Request for Proposal to participate in 2026 will open 5 December, with the 12-month project commencing March.

Grant funding will be available for up to five practices within each of WAPHA’s Primary Health Network’s (Perth North, Perth South and Country WA).
