New directory supporting the South West to eat well and be active

WA Primary Health Alliance and WA Country Health Service Southwest have partnered with My Community Directory to develop the South West Healthy Lifestyles Directory.

The free, online directory links people living in the South West region of WA to local services and activities to support them to eat well and be active.

A collaborative initiative of the South West Healthy Lifestyles Working Group, the Directory includes a variety of not-for profit and MBS subsided services such as dietitians, physiotherapists, and exercise physiologists, along with recreation centers, sporting clubs and community groups.

Health professionals can use the Directory to refer patients and clients to relevant support services and can request to be featured by contacting My Community Directory.

Directory and SHAPE website launched in the South West

Health professionals in the South West attended the joint launch of the South West Healthy Lifestyles Directory, SHAPE website and Conversations About Weight online training suite, to learn how these new resources can support them to assist their patients with weight concerns to improve health and wellbeing outcomes.

The SHAPE website contains tools, resources, and education for general practice and primary healthcare professionals, to support a better understanding of the factors influencing weight, and how to engage patients and guide them on their journey. Through this we can drive long-term change.
