Older people in the Goldfields, Gascoyne and Pilbara who need extra support to access aged care will benefit from an expansion of the Australian Government-funded care finder program, commissioned by WA Primary Health Alliance (WAPHA).
For the first time, Amity Health will deliver the service in the Goldfields-Esperance region, while Mission Australia will deliver the service in the Gascoyne and Pilbara regions. Additionally, Mission Australia will deliver the existing care finders service in the Midwest.
Care finders helps vulnerable older people connect with aged care, health, housing and community supports that enable them to remain safe, independent and connected to their communities.
The service provides practical, one-on-one assistance to people who are unable to navigate the aged care system on their own and do not have family or friends available to support them.
In Western Australia, 1,851 clients accessed care finders in the past year, with more than 8,000 care finder services delivered between July to December 2025.

WAPHA CEO Bernie Kenny said the expansion would help reduce barriers to care for some of the region’s most vulnerable community members.
“Everyone deserves access to the care and support they need as they age, regardless of where they live. Expanding care finder services into the Goldfields, Gascoyne and Pilbara regions will help more older people connect with aged care and other essential services, reducing the risk that they fall through the cracks.
“Through trusted local providers Amity Health and Mission Australia, this program will deliver practical, personalised support that makes a real difference to people’s lives and wellbeing.”
Amity Health CEO, Pam Bubrzycki said the organisation welcomes the opportunity to expand care finder services into the Goldfields and across the Wheatbelt, building on the strong foundations established over the past three and a half years in the Great Southern and parts of the Wheatbelt.
“Through our ongoing partnership with WA Primary Health Alliance, we can reach more older people who may otherwise struggle to access aged care and community supports. This investment will help ensure vulnerable people in regional communities receive the practical assistance they need to navigate services, maintain their independence and improve their quality of life.”
Mission Australia State Director WA, Elise Jorgensen said the organisation is thrilled to be the care finder provider across the Midwest, Gascoyne and Pilbara regions, strengthening its commitment to supporting older Australians to access the care and services they need to live well and remain connected to their communities.
“We recognise that navigating the aged care system can be challenging, particularly for those experiencing vulnerability or social isolation. Our team look forward to providing personalised, face-to-face support that helps people overcome barriers, access appropriate services and maintain their independence and wellbeing.”
The care finder service is available to people aged 65 years and over, Aboriginal and Torres Strait Islander people aged 50 years and over, and people aged 50 years and over who are on a low income and homeless or at risk of homelessness.
To receive care finder support, a person must need intensive assistance to understand and access aged care services and connect with other supports such as health care, housing services and social supports. This may include people who are socially isolated, have communication or cognitive challenges, or have had previous experiences that make it difficult to engage with services.
This service is supported by funding from the Australian Government under the PHN Program and is commissioned by WA Primary Health Alliance.
